How to Register for a Sport
Registration is completed online through FinalForms. Your information is saved from season-to-season and year-to-year, making the registration process quick and easy. Registration for each season of sports will open approximately 4 weeks prior to the beginning of the season.

How to Register for a Sport
- Visit https://lakewashington-wa.finalforms.com/
- Click LOGIN under the Parent icon.
- Click INCOMPLETE FORMS.
- Select the sport for which you would like to register. Register for only one sport per season.
- Complete each form and sign your full name in the Parent Signature field at the bottom of the page. After signing a form, click SUBMIT FORM and move on to the next form.
- You will see a “Forms Finished” message when all forms are complete.
- An email will automatically be sent to the your students LWSD email account, prompting them to complete the forms requiring their signature. Parents, please do remind your student to sign their forms! If students can't access their LWSD email account they can log in directly to FinalForms and sign.
Physical | Medical Eligibility Forms
All student athletes must have a valid physical exam, using this form: Preparticipation Physical Evaluation - Physical Examination Form.
Completed forms can be uploaded directly into FinalForms as a pdf. To upload physical to FinalForms: Log into FinalForms and select the WIAA PPE PHYSICAL form, directions to upload are on this form.
Please note: Doctors signature and clinic stamp are required on both pages of the form. Please review your forms prior to submission to make sure they are complete!
Wondering when your students physical expires? Log into FinalForms and you will see the date listed below your students name.
Fees
All students participating in a high school sport must purchase an ASB card. These funds directly support the year-to-year cost of the athletic programs, such as: facilities, equipment, uniforms, game transportation costs, and more. If you attend a CHOICE school and live in the LWHS boundary you will also be required to purchase an LWHS ASB card.
There is a $250 LWSD Participation fee for each sport. Fees are collected on a per sport basis with both individual and family caps. The family caps will apply for brothers/sisters who attend the same school. Fees must be paid at the time of turnout (after cuts are made for try-out sports). High school parents can pay student fees online or arrange with the LWHS Bookkeeper to pay in person with cash, check or credit card (no Amex).
Families that qualify for Free and Reduced meals, may be eligible for a free ASB Card and to have sports participation fees waived. Apply here.
2023-2024 Athletic Participation Fees
- $250 per sport
- $500 individual cap
- $750 same school family cap
100% refund will be provided under the following conditions:
- Student is cut from the squad by the coach
- Student quits due to illness or injury prior to first contest
- Student quits due to family moving prior to first contest

Sports Only Students | Running Start, Choice School, Private, Homeschool, Online
Choice School Students
If your student is Currently enrolled in a Lake Washington School District Choice School you must re-register/activate your student with their neighborhood school as a Sports Only Student each year. Please contact JoDee Hull to do this.
To access registration in FinalForms, please email JoDee Hull with your name and what sport (s) you are registering for. Mrs. Hull will then add you to the roster making Finalforms available.
Once your selected sport is added by Mrs. Hull you will be able to access the sport specific forms in FinalForms.
Reminder, along with the LWSD Participation fee for your sport you will also need to annually purchase an ASB card ($60). If you are trying out for a sport you will not pay for an LWHS ASB card until after the try-out.
Private, Online & Home School Students
Students attending Private, Online and Home School programs are eligible to participate in athletic programs at their neighborhood school as a Sports Only Student.
Reminder, along with the LWSD Participation fee for your sport you will also need to purchase a LWHS ASB card ($60).
If your student has never registered with Lake Washington School District before:
First- You must register your student with your Lake Washington School District Neighborhood School.

Next – Setup your Family Access Account (Skyward)
You will receive an email from the school district to access your Family Access Account which is also referred to as your Skyward Account. (please note that it can take up to 2 school days for your student to show ‘in the system’.)
Then – After your Skyward account is set up you will get an email from Final Forms promoting you to set up an account. Follow directions on the Athletics page for Athletic Registration.
Private & Online School Students
If you are enrolled in any off-campus classes, complete the Alternative Education and WIAA Eligibility contract, print your class schedule, and return to the athletic office either in person or via email.
Home School
If you are home schooled, complete the Home Based Education and WIAA Eligibility contract, print your class schedule, and return to the athletic office. You will need to provide grades on grade check dates during the season in which your student is playing.
Grades
Grade checks are run throughout each sports season. Please provide a copy of previous semester and most recent quarter grades. Failure to provide grades may make you ineligible for competition.
If you attend any other school for ANY classes, you must provide the Athletic Office with your grades from that school.

Students are required to complete a WIAA contract if they do not attend the school full-time.

LWSD Athletic Grade Policy
LWSD Eligibility Criteria
Grade checks are run throughout each sports season. You must be passing 6/7 classes to participate fully in athletics.
- At the end of the PREVIOUS SEMESTER, fall student-athletes in grades 10-12,who do not meet the WIAA academic standard (failing more than one class) are ineligible through the fourth Saturday of September.
- A student may earn a “class-for-class” replacement earned in a regular, accredited summer school program or alternative educational program accepted by the school district.
- Per WIAA rules, incoming 9th grade students do not have to meet an academic standard to be eligible for the initial Fall grade check, other than having a previous semester of attendance. Incoming 9th grade students first grade check will be in early October.
If you attend any other school for ANY classes, you must provide the Athletic Office with your grades from that school by the day of the grade check.
